The first part of this page contains the information about the variable from the CPHN Guide for Common Indicators for Canadian Physician Health Programs, version v1.0. CPHN is editing the guide and will eventually produce a new version, v1.1. Concurrently, we are also writing a new version of the guide, for use with CiviCase. We will call this stream v2.0.
Service Provided

This section records services provided by the PHP. Several different services may be provided to an individual or client, and all should be recorded in the format below. This variable includes only services provided or actions taken directly by the PHP, not those for which an individual or client is referred elsewhere. For example, if a client is given financial advice by the PHP, record it here; if a client is referred elsewhere for this service, record is here as "Referred elsewhere" and record the specific service values in Where referred to for service.
