Generic intake of a client

__Story:
Client walks into a service provider.
Service provider staff gathers information about the client and enters it into CiviCRM.

__Reports:
1. New intakes
Shows unique ID of client, name, contact information, intake staff member, status of the intake

2. Intakes by relevant criteria
Intakes by program
Intakes by diagnosis
Intakes by demographic

__CiviCRM use case
1. Create new individual
2. fill out custom field groups for relevant client information
3. create a new case record for each service (intake, class, appointment) ????
