user logs in
user locates an existing case
user creates a new activity
- system asks you to choose an activity type
user chooses "close case" as the activity type
- system opens a blank activity
- date field will show label of "effective date of case closure"
- choice field is available for editing showing list of primary reasons for case closure.
user indicates the effective date for case closure
user indicates the primary reason for case closure
user saves the activity
- system updates the cached variables about the case to show that its status is now "closed" and the date of closure = the date the user indicated as "effective closure date".
